										<content:encoded><![CDATA[<p class="wp-block-paragraph">Giving birth, it only takes a few days/hours. Yet it is these hours that have a huge impact. Because from that moment, your life changes forever.</p><p class="wp-block-paragraph">Suddenly you have the baby you have dreamed and hoped so much about in your arms. How wonderful.<br>But... practice is sometimes more unruly. Many women do not look back on their delivery well. And even though it was short, these few hours have a huge impact on your life afterwards. Especially if you don't look back on this experience properly.</p><p class="wp-block-paragraph">It is well known that the reasons why women do not look back on their childbirth well can vary widely. Women with 2 almost identical deliveries can still look at that event very differently. For instance, I remember 2 ladies whom I spoke to within 1 week in their childbirth. They had both had vacuum deliveries. One of the ladies really struggled with this, was extremely upset and felt very sad about it. The other lady had had a top delivery. Too bad she needed a vacuum, but it was for the best, so she was at peace with that. She looked back on her delivery very well.</p><p class="wp-block-paragraph">When I heard that, I was still a student. And at that time, I realised that trauma from childbirth depends on many factors.</p><p class="wp-block-paragraph">I ran into it again last year. I had a follow-up check-up of a woman with whom I had done a beautiful home birth. She did not want to give birth at home, but there was no time left to go to the hospital.</p><p class="wp-block-paragraph">What seemed like a dream birth for me was very traumatic for her. She really hadn't wanted it this way. Although she was happy with my guidance, she definitely did not want to give birth this way again.&nbsp;</p><p class="wp-block-paragraph">It is well known that women can suffer PTSD as a result of childbirth. This is a reaction to a traumatic experience or life-threatening event. Symptoms of this can include reliving the situation, avoidance of situations that remind of the event and irritability.</p><p class="wp-block-paragraph">What we also know is that this trauma can be so intense that women do not want to go through it again and let it determine their family and desire for children.</p><p class="wp-block-paragraph">There are a number of factors that can play into this. Factors that make women perceive their childbirth as traumatic:</p><ul class="wp-block-list"><li>Loss of control and dignity</li><li>Hostile/annoying attitude of people around you during childbirth</li><li>Not being heard or not being explained about medical intervention during your delivery</li><li>Acts performed without your consent (going against your wishes and boundaries)</li></ul><p class="wp-block-paragraph">Medical complications during labour can also cause labour to be perceived as traumatic.</p><p class="wp-block-paragraph">You can never rule out a traumatic birth because we cannot see into the future. There are things that can come into play, though.</p><ol class="wp-block-list" type="1"><li><em>Find people around you who support you.</em>&nbsp;Your own midwife: it is important to choose a midwife you trust and who you feel knows your wishes and stands up for your rights. It can also be a doula, someone who supports you throughout labour. Your birth partner, a family member or friend.</li><li><em>Prepare well.</em>&nbsp;It's good to know how childbirth can go. And to see or read great delivery stories. But sometimes practice is more unruly and things are more complicated. It can help you to know a bit more about that too. Some people like to have a detailed birth plan, while others let things take their course. Everything is fine as long as you feel comfortable with it.<br>Taking a childbirth course can help. This will give you insight into the course of childbirth. It is also good for your birthing partner to know what you will need during labour. Do you want people around you to be quiet or encourage you? Do you need music or affirmations? Would you like a back massage?</li><li><em>Put your wishes on paper.</em>&nbsp;I also mentioned this point above. For some people this is not necessary, they let it all come to them. But if you know you have specific wishes, it can be useful to put them down on paper. That way, during your contractions, you don't have to "fight" for how you would like your labour. It can also help to make a birth plan for specific situations. For example: home birth, introduction, caesarean section.<br>Especially if you have a care request outside the guideline, it is also good to put it on paper and discuss it beforehand with your midwife or at the hospital.</li></ol><p class="wp-block-paragraph">You may be reading this and recognise it from your own childbirth. That can be quite sad.</p><p class="wp-block-paragraph">Even then, I have some tips for you.</p><ol class="wp-block-list" type="1"><li><em>Find a healthcare provider that suits you.&nbsp;</em>You may not have felt supported during your labour. If so, find another midwife who suits you. And consider a doula*. Because trust in those who will be at your birth is crucial. You need to be able to surrender completely to their good care when you have contractions. Sometimes it is necessary to switch midwives' practices.</li><li><em>Engage in conversation.</em>&nbsp;This can be done in several ways. If you find that you cannot let go of your birth, you can have talks with a psychologist to process your birth. (EMDR is a therapy used for this, among other things).<br>But it may also be good to request a preconception consultation. A conversation with an obstetrician or gynaecologist before you are pregnant. In this conversation, you discuss what has been traumatic for you and how you would like to avoid this next time. &nbsp;<br>If you are pregnant, it is good to have a conversation about this with your healthcare provider at the time you need it. Many hospitals and midwives usually schedule this conversation around 34-36 weeks. But you may well need it much earlier. Check with your healthcare provider what you need to make this birth a positive one.</li></ol><p class="wp-block-paragraph">So there are things that can help prevent childbirth trauma.</p><p class="wp-block-paragraph">Should you struggle with this, do not hesitate to contact us. 										<content:encoded><![CDATA[<p class="wp-block-paragraph">Nausea in pregnancy, it's part of the deal. Right...? It is often one of the first signs you can tell you are pregnant. Women who don't feel nauseous at all are sometimes unsure, because yes, how do you know in the first few weeks that your pregnancy is okay?</p><p class="wp-block-paragraph">Is nausea actually that normal? How long is it supposed to last? And when is it no longer normal?</p><p class="wp-block-paragraph">In this blog, I will explain more about this to you. But first, I will tell you my own story.</p><h2 class="wp-block-heading">My story</h2><p class="wp-block-paragraph">In 2015, I got pregnant with my daughter. My husband and I were extremely excited. Early in the pregnancy, at around 6 weeks, I started getting quite nauseous and vomiting. Soon, this increased very much and I was vomiting all day. Literally every drop I ingested came out within 5 minutes. But yes, I thought the step to seek help was quite big. Eventually, I called my midwife (I was not a midwife myself at the time) and ended up at the hospital quite dehydrated and weakened. The nurses tried to inject an IV, but my blood vessels were too empty due to all the fluid loss.  After nine attempts, the OR managed to stick an IV in my elbow crease. Hyperemesis gravidarum, or excessive vomiting, was the conclusion.</p><p class="wp-block-paragraph">I ended up being hospitalised 3 times in the first 12 weeks. I was hugely nauseous, tired and debilitated. And I was thirsty. Thirst like you can't imagine. Such thirst that I drank, even though I knew it would come right back out. But the thirst took hold of me completely. I had to and would drink. I even got a nurse in hospital once to make my drip faster. That was not allowed because of the side effects, but I had to and would get more fluids.</p><p class="wp-block-paragraph">At 12 weeks, I hit for the 4<sup>e</sup>&nbsp;dehydrated once, but now I managed to keep something down. I drank about 1 litre a day then, but still spit up a lot. A reason for the hospital not to admit me then.</p><p class="wp-block-paragraph">This period was heavy and intense. The nausea and weakness got so bad that I couldn't do anything else, all my activities had to stop. Even my husband had to adjust his rhythm to my being sick and hospitalisations.</p><p class="wp-block-paragraph">What kept me going was that I was immensely grateful to be pregnant. A fact that not everyone is granted. That put my own situation into perspective.</p><p class="wp-block-paragraph">Even with the pregnancy of our second child, I became nauseous again around 6 weeks of pregnancy. I was spitting up daily, often. But I kept more inside than I spit up. It was good enough. Until I was 12 weeks pregnant. Then I had to report back to the hospital after 36 hours of spitting up.</p><p class="wp-block-paragraph">For me, a huge improvement. From a pregnancy with 3 admissions of a few days to a pregnancy with 1 day in hospital.</p><h2 class="wp-block-heading">The facts</h2><p class="wp-block-paragraph">Research shows that 50-80% of pregnant women feel nausea during pregnancy. This often passes between 12 and 16 weeks of pregnancy.</p><p class="wp-block-paragraph">Symptoms that may accompany it include vomiting or the tendency to vomit, sensitivity to certain smells and tastes, heartburn and fatigue.&nbsp;</p><p class="wp-block-paragraph">Some women vomit so much that they cannot stop vomiting and it seriously disrupts their daily lives. An admission is then needed to regain enough fluids and gain strength.</p><p class="wp-block-paragraph">There is no single definition of hyperemesis gravidarum. Some features may include nausea, extreme vomiting, dehydration, inability to eat and/or drink, extreme fatigue, more than 5 % of body weight loss, inability to do your daily activities.</p><p class="wp-block-paragraph">This can occur in 0.5 - 2% of pregnancies. It seems to be more common in girl pregnancies (was definitely the case with me, with my daughter I was much more nauseated than with my son) and twin pregnancies. Heredity may also play a role. &nbsp;</p><h2 class="wp-block-heading">Emotional impact</h2><p class="wp-block-paragraph">Pregnancy sickness, even if milder, can have a huge impact. Many women find it difficult to be happy about the pregnancy and struggle to get on with everyday life. Because it often happens at the beginning of pregnancy, women often do not want to tell people they are pregnant yet. As a result, they receive less support from those around them.</p><p class="wp-block-paragraph">When women do tell, they experience misunderstanding. "Nausea is just part of it", or "try eating small bits and try ginger (tea)" are common comments. Well-intentioned, but these tips don't work if you really can't keep anything down. Such extreme nausea is an illness and not a normal pregnancy complaint.</p><p class="wp-block-paragraph">For many women, nausea is a reason not to start another pregnancy or later.</p><p class="wp-block-paragraph">Pregnant women also often feel misunderstood by healthcare professionals. For example, weight loss and ketones in urine may be looked at. But with extreme nausea, sometimes only fatty things like chips manage to stay inside. This causes women to have less weight loss and no ketones in urine. As a result, a doctor may mistakenly think it won't be too bad.</p><p class="wp-block-paragraph">It is important to find people around you that you feel supported by. Who acknowledge that it may be part of your pregnancy, but that doesn't make it normal yet. It is also important to get enough rest and seek help if necessary. This can ensure that you are, perhaps temporarily, able to move on again for a while.</p>]]></content:encoded>

										<content:encoded><![CDATA[<p class="wp-block-paragraph">There has been a lot to talk about in midwifery land recently. The birth movement has even launched a campaign to draw more attention to birth trauma and even birth violence. Why is it that so many women experience childbirth as traumatic? And is there anything you can do yourself? The professional association of gynaecologists recently posted an article about this, they believe it is mainly due to unrealistic expectations. If then these expectations cannot be met, women are disappointed and are more likely to experience childbirth as traumatic. Of course, it is very important to get good information about what to expect during childbirth. You can also prepare yourself well for this by reading books or taking a course. But research shows that there is an even more important reason why women experience childbirth as traumatic: lack of control. During childbirth, nature does its work in all its glory and it is a very delicate process of hormones, acceptance and safety. A disruption of this process can affect the course of childbirth. Some things you can influence and others you cannot. I have never had women in practice who had the expectation that they could control the birth and that it was going to go exactly as they had planned. Everyone is aware that sometimes things happen during childbirth that are beyond your control. But then what could make the difference. For one thing, good preparation is very important. Knowing what to expect and all the choices you can make helps to maintain your sense of direction. On the other hand, you can ask questions during your delivery if a healthcare provider suggests certain policies. Ask about the alternatives and then make a conscious choice. You don't always have to follow the path prescribed by the protocol. Sometimes situations arise where it can actually be helpful not to follow the protocol. This way, you keep control over your delivery. You can apply this principle everywhere, including during your pregnancy. In consultation with Myrthe, I made other choices during my birth than the protocol would prescribe. At my request, for instance, she only did an internal examination once. This because it went well and the number of centimetres didn't change anything in the policy. Thus, we made a conscious choice together. To prepare yourself for the policy during labour, but also during pregnancy and puerperium, you can find information at vraagdevroedvrouw.nl. This website was set up by an obstetrician who made an overview of all the research that has been done on various topics. So you can read on what basis certain protocols have been drawn up and then, when faced with the choice, make a conscious choice. 										<content:encoded><![CDATA[<p class="wp-block-paragraph">Early on in pregnancy, we usually ask what food you want to feed your baby. Let's say up front that no choice is the wrong choice. I wanted to breastfeed because I always try to take nature as much as possible as my starting point. I also relied on that during my delivery and I wanted to do the same with feeding. Nature has thought about that too and the nutrition you make as a mum is completely adapted to your baby's needs. Through the saliva your breast comes into contact with, your body adapts the nutrition and passes on antibodies against any viruses found in the saliva. This is how your body protects your baby. How nice is that! Of course, every advantage has its disadvantage. When it comes to feeding, too, it is not all plain sailing and sometimes it can be a struggle. Some babies refuse the breast, others refuse the bottle. It is always a search for the way that suits you and your baby. Being a breastfeeder myself, I can only share this experience with you. After giving birth, I wanted Maas to drink at the breast within an hour. This is important for the first stimulation, the bonding between mother and baby and the first signal to your body for the production of nutrition. Even if you are not breastfeeding, it is important for the bonding between mother and child that your baby lays on your breast for the first hour. Because my delivery went without complications, I also had this opportunity. Maas turned out to be a natural and latched on well right away! In practice, I have seen several times how much energy it can take to get a baby to latch on properly. So I had also prepared myself for the fact that this would also be the case for me. This is because it takes a while for your milk flow to get going properly. This usually only happens around 3-4 days after delivery. You notice this because you get congestion, or as we call it: Pamela day! For the men: you can look, but don't touch! So the milk flow gets going well that day and this results in very full breasts! They feel hard and tense. Fortunately, this usually lasts about 24 hours and then it subsides again. Your body then starts adjusting the amount of milk to your baby's demand. You will also see that from then on, your baby will start growing again. In the beginning they lose weight because the milk flow is not yet established. They have gained extra fat reserves for this so they can get their energy from this during the first few days. So your baby is adjusted to the fact that it will take a while for them to get milk. With me, fortunately, this all went smoothly but this is by no means a given! This is why we are very grateful that, as midwives, we can call in the lactation consultant! These are people who have completely immersed themselves in feeding your baby and have learned how to deal with problems with this. So, as you have read earlier, Maas has reported every two hours from the start. I did find this tough, but fortunately by pumping I was able to share the workload with Jordy. Now I am now very happy that it worked out with breastfeeding. I especially find it very convenient that I always have food with me, at the right temperature and in the right quantity! 										<content:encoded><![CDATA[<p class="wp-block-paragraph">Parturition pain, the thing most women dread. What does a contraction feel like and can I handle the pain. This is something you wonder especially before a first birth. We always say: as long as you are in doubt whether it is a contraction, it is not a contraction. Often these are pre contractions that are definitely already painful but do not yet cause dilation. As you have read in my previous blog, I have also been plagued by these contractions. What exactly a contraction feels like I can't tell you. Not only because you forget ( yes it's really true......). Also because every woman experiences contractions differently. If we start looking at the biological process of contractions, there are two main hormones that play a role in making contractions happen. Oxytocin and endorphins. These two hormones allow your body to make contractions and also ensure that your body can cope with them. There is also a hormone that can interfere with this process: Adrenaline. Your body produces this hormone when you experience stress. In practice, I always take the following example: Your body does not know where you are going to give birth, for the same money you are somewhere in the jungle. Your body gives you birth pains so you know you have to find a safe place (so you are in the jungle with all its dangers and not in your safe house). If you don't feel safe, your body produces adrenaline to indicate that you are not safe enough to give birth, the result: the contractions weaken and your body stops labour. When you feel safe and relaxed, your adrenaline hormone is little to nonexistent and your body does not receive a stop signal. So your body can freely produce the hormones needed for good contractions: Oxytocin for contractions and endorphins for pain relief. How you ensure that your body can produce these hormones properly is very personal for everyone. During training, for instance, we did a training to discover what helped us deal with pain. We had to hold an ice cube in our hand and try out different things each time. For instance, we had to put on happy music or relaxation music and we had to massage each other to find out what helped against pain. So to prepare yourself well for childbirth, you can go on a journey of discovery to find out what helps you when you are in pain. For me, for example, that was relaxation and being alone. I had made a playlist beforehand with music that made me feel relaxed and had a diffuser with lavender oil on. Furthermore, it helped me to tell myself with each contraction that the pain was okay and that it was going to help me. In the end, there are often two moments in labour when you think you can't cope. Usually this is around 2-3 cm and around 9 cm. Prepare for these moments and discuss what you need with the people attending your labour. Giving birth is about letting go and surrendering to your body and nature.
